Job Title: Medical Scheduler Case Manager Assistant for Transition of Care


Location: San Francisco, CA


Job Description: LHH Recruitment Solutions is seeking a dedicated Medical Scheduler Case Manager Assistant for an established healthcare client in San Francisco, CA. This onsite role ensures seamless transitions for ambulatory patients from hospital discharge to post-acute care settings. The position is full-time, 8 hours a day from 08:00 to 17:00, for a duration of 26 weeks with the possibility to extend. Key Responsibilities:



  • Collaborate with Case Managers and Social Workers to develop and implement Transition of Care (TOC) plans.


  • Communicate with patients and families about post-acute care options and discharge plans.


  • Monitor and document progress towards TOC goals, addressing any barriers.


  • Coordinate post-discharge referrals and authorizations.


  • Facilitate patient transfers to post-acute facilities, including preparing documents and arranging transportation.


  • Participate in daily team huddles and department meetings.


  • Maintain accurate documentation in the electronic medical record system.


  • Provide clerical support as needed.


Qualifications:



  • High School Diploma or equivalent education/experience.


  • Minimum of 1-year recent relevant experience.


  • Strong communication, interpersonal, and time management skills.


  • Ability to work effectively in a fast-paced environment with shifting priorities.


  • Proficiency in PC and word processing skills.
