- Provide comprehensive case management services to individuals with SMI, ensuring their needs are met and they receive appropriate care and support.
- Conduct assessments, develop individualized care plans, and coordinate services with healthcare providers, social services, and community resources.
- Perform field work, including home visits and community outreach, to monitor clients' progress and provide support in their natural environments.
- Advocate for clients' rights and ensure they have access to necessary resources and services.
- Maintain accurate and up-to-date documentation of client interactions, care plans, and progress reports.
- Collaborate with a multidisciplinary team to deliver holistic and client-centered care.
- Bachelor's degree in social work, psychology, or a related field.
- Previous experience working with individuals with SMI is required.
- Strong understanding of mental health issues and community resources.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation for field work.
